We are delighted that you have chosen the University of Sunderland as your firm choice of institution and look forward to you studying with us.
As you are probably already aware, the programme you have chosen to study involves working with children or vulnerable adults. Below you will find all the necessary instructions and forms you will need to download, complete and return in order to meet the necessary clearances for your chosen programme.
Please note: you are required to submit a Criminal Records Bureau (CRB), this will be sent to the address you have provided on your application, you must gain CRB clearance via the University of Sunderland. Previous CRB clearances will not be valid.
Please carefully read the instructions before completing the CRB and Medical Questionnaire forms. Any incorrect or incomplete forms will not be processed and may delay your intake onto the programme.

Social Work Interview Information
Prior to your interview you must download and complete the Social Work Declaration form on the link below. For further information with regards to completing this form please refer to your interview letter.
