Request for Access to Personal Data under the UK GDPR and Data Protection Act 2018

This form is for individuals wishing to request access to personal data held about them by the University of Sunderland, in accordance with the UK General Data Protection Regulation (UK GDPR) and the Data Protection Act 2018. Please complete all relevant sections to help us process your request efficiently.

Copies of evidence and identification should be sent via email to dataprotection@sunderland.ac.uk, following completion of the Subject Access Request form.
